Person-Centered planning (PCP) is a set of approaches designed to assist someone to plan their life and supports. It is used most often as a life planning model to enable individuals with disabilities or otherwise requiring support to increase their personal self-determination and improve their own independence.

PCP was created in response to some specific problems with the way in which society responds to people with disabilities. PCP discovers and acts on what is important to a person. It is a process for continual listening and learning, focusing on what is important to someone now and in the future, and acting on this in alliance with their family and their friends. It is accepted as evidence-based practice in many countries around the world.
